On October 12, 2011, the Mommy & Me Program of Hilliard Chapel received a $25,000 grant from Sierra Health Foundation through its Responsive Grants Program.  The grant was one of 28 awarded by the Sacramento-based private philanthropy, which promotes improved health and quality of life for the people of Northern California.

The Mommy & Me Program, a faith-based community organization of Hilliard Chapel, was established in 2008 to reduce the high rate of Black infant mortality, eliminate ethnic and racial healthcare disparities, and empower low-income women in San Joaquin County to achieve healthy pregnancies and healthy newborns.  This grant allows Mommy & Me to continue its work in the low-income pregnant and parenting community to provide services that have been eliminated or reduced as a result of the current economic environment.   Mommy & Me partners with the local community to bring free services to the low-income, underserved population of San Joaquin County.

Mommy & Me focuses on the elimination of healthcare disparities and the reduction and elimination of infant mortality in San Joaquin County where the rate of Black infant mortality is 16.1 for every 1,000 live births and where the rate of premature and low-birth weight babies is more than twice the national and state averages.

In addition to providing parental and well-baby education and resources, Mommy & Me operates an on-site boutique that consists of new and gently used infant and toddler apparel, infant formula, disposable diapers, blankets, maternity wear, and personal hygiene products. Clients of the Program receive Mommy & Me Bucks for each positive action taken toward health education and self-empowerment. The Mommy & Me Bucks collected allow a pregnant or parenting woman to purchase items for herself and her baby from the Mommy & Me Boutique.

Sierra Health Foundation awarded a total of $571,731 to support projects that improve health and quality of life in the second 2011 Responsive Grants Program funding cycle.  Through the Responsive Grants Program, Sierra Health Foundation asks nonprofit organizations and public agencies how they can promote and improve health in their communities.  Since launching the program in 2008, Sierra Health Foundation has awarded more than $4 million to provide support for 166 programs and projects.
